29 CFR 1910.305(g)(1)(i): Flexible cords and cables were not approved or conditions of use and location:      On or about November 4, 2013, and at times prior thereto, employees were exposed to an electrical shock and fire hazard while using an Utilitech flat plug cord (models-523S) plugged into an Axis power strip (model YLPT-2) while working at the kiosk in Ingram Park Mall.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      On or about November 4, 2013, and at times prior thereto, employees were exposed to an electrical shock hazard by using a power cord that had strain relief damage to the outer sheathing at the kiosk in Ingram Park Mall.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:      On or about November 4, 2013, and at times prior thereto, employees were exposed to an electrical shock hazard by using a power cord that had been cut, then repaired with electrical tape and duct tape at the kiosk in Ingram Park Mall.
