29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access. a) West Side of the Site - Two masons were observed on a tubular welded frame scaffold without a ladder for access exposing them to a 13 foot fall hazard, on or about 10/21/13. This contractor is the general contractor for the site, thusly; they are the controlling and correcting employer for the site. The company Vice President and Project Manager stated they conducts daily inspections and are continuously monitoring all operations. The Project Manager stated in his interview that he was aware of the operations in progress. Therefore, this contractor is being cited Under the Multi-Employer Workplace Policy.
