A worker was exposed to being struck by falling objects such as shingles, hammers, and nail guns while on the ground cleaning up debris below roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Jobsite: Sidestreet elevation.    Employees were exposed to a fall of approximately 22 feet to the ground below while accessing the roof from an aluminum extension ladder whose siderails did not extend at least 3 feet above the eave.

Three (3) employees were exposed to a fall of approximately 22 feet to the ground below while performing roofing work where fall protection was not utilized.    Lema Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315141879, citation number 2, item number 1 and was affirmed as a final order on August 15, 2011, with respect to a workplace located at 756 Granite Street, Braintree, MA.    Lema Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 312340730, citation number 1, item number 1 and was affirmed as a final order on March 22, 2009, with respect to a workplace located at 114 Summit Street, Pawtucket, RI.
