On or about 25 October 2013, two employees were performing plastering work on a three-tier tubular welded frame scaffold without the use of the toeboards where materials and mixing buckets were located on the second and third platform of the scaffold while one of the employees was underneath, exposing the workers to the hazards of being struck by debris or materials.
