29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Worksite - Employees working directly below ongoing roof work were not protected by protective helmets; on or about 11/05/13. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Worksite - Employees were using an unsecured ladder to access the roof. The ladder side rails were approximately 14 inches above the upper landing; on or about 11/05/13.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Worksite - Employees replacing an approximate 8-12 pitch roof approximately 12 feet above the ground were not protected from falling; on or about 11/05/13.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.      Idrovo Roofing & Siding, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 315608778, citation number 1, item number 1 and was affirmed as a final order on 10/6/11, with respect to a workplace located at Rte. 46 E. & New Rd., Parsippany, NJ 07054.
