29 CFR 1910.212(a)(1): Guards on machines were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:     On or about November 5, 2013, and at times prior thereto, an employee in the spice mixing room was exposed to an amputation hazard while using a Univex M60 mixer without a guard for the rotating auger.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:  On or about November 5, 2013, and at times prior thereto, an employee in the spice mixing room was exposed to an electrical hazard while using a Univex M60 mixer that did not have strain relief on the 220V flexible power feed cord.
