Gabriel Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 108277, citation number 2, item number 1, and was affirmed as a final order on May 6, 2013, with respect to a workplace located at 6125 West Bradley Ave, Milwaukee, WI 53223. Gabriel Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 313720112, citation number 1, item number 1, and was affirmed as a final order on August 17, 2010, with respect to a workplace located at 6670 Lonewood, Colorado Springs, CO 80925. The leg straps on the employees harnesses were not fastened and the harnesses were not attached to the lanyards/anchor points. Gabriel Lopez Alvarez dba Three Amigos was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA inspection number 108277, citation number 2, item number 2 and was affirmed as a final order on May 6, 2013, with respect to a workplace location at 6125 West Bradley Ave, Milwaukee, WI 53223.
