29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system:       a) Jobsite, 421 Main Street, Youngstown, Pennsylvania 15696:  Observed on or about November 05, 2013, employees engaged in roofing activities on a low slope roof (2 in 12 vertical to horizontal) with unprotected edges were not protected from falling.  The employees were exposed to a fall hazard of approximately 10 feet to the ground below.

29 CFR 1926.503(c):  The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:       a) Jobsite, 421 Main Street, Youngstown, Pennsylvania 15696:  Observed on or about November 05, 2013, the employer did not provide retraining to employees who demonstrated an inadequate understanding of recognizing fall hazards and the procedures to be followed in order to reduce or eliminate these hazards.  The employees were observed working in close proximity to the edge of the roof while not protected from falling.  The employees were exposed to a fall hazard of approximately 10 feet to the ground below.
