29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about November 1, 2013, at 126 Churchill Rd, Boerne, TX, employee exposed to a four feet fall hazard. Employee was observed stepping on the top rung of the step ladder and then again was observed leaning the ladder against the wall and then climbing it.  The Werner step ladder was used in a manner that is prohibited by the manufacturer.  In addition, 29 CFR 1926.1053(b)(13): The top step or top of the ladder shall not be used as a step.
