29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) 7001 North Park Drive, Pennsauken, NJ 08109 -  The employer did not develop and implement a written lockout/tagout program that provided for the training of authorized and affected employee, and an annual determination that authorized employees were following the established procedures. Employees were exposed to energy release hazards while servicing and maintaining machines that included but were not limited to shears, die cutters, and printing presses. On or about 11/04/2013.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury were not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a) Die Cutting Department -  The point of operation of the Heidelberg "Windmill" press was not guarded. It was possible for an employee to have body parts in the clamshell part of the machine while it was cycling. On or about 11/05/2013.  b) Die Cutting Department -   The point of operation of the Heidelberg Cylinder Press was not adequately guarded. It was possible for an employee to have body parts in the area between the rotating cylinder and me machine frame while the machine was cycling. On or about 11/05/2013.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
