29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  a) Location - Wood Shop:  On or about November 1, 2013, the floors, building components and area inside and above the make shift spray area had heavy accumulation of wood dust and adhesive overspray exposing employees to fire hazards.      b) Location - Painting Department:  On or about November 1, 2013, the areas far outside of the powder coating spray booth had large accumulations of overspray on the building horizontal  and vertical (wall) surfaces ,including the gas fired oven immediately adjacent to the spray booth.   Employees are exposed to potential fire or potential combustible dust hazards.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a) Location - Production:  On or about November 1, 2013, where noise levels were elevated during production (equipment or processes) that may harm employee hearing, the employer did not conduct noise monitoring or surveys to determine if the OSHA limits were being exceeded. In the metal department, operations such as metal cutting and grinding, noise levels measured by sound level meter were 93-97 decibles.  Personal exposure monitoring of grinding metal parts showed average level of 95 decibles for time sampled.
