On or about November 5, 2013, employees using the JLG personnel lift to access the roof were not utilizing fall protection while riding in the lift.

State St. in DuBois, PA. On or about November 5, 2013, employees installing the new roof to the building, 28 feet above the ground, were not using fall protection while working on the roof.
