29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about November 4, 2013 and times prior thereto, employees of Stanley Lantzsch Contracting performed framing and decking work at a construction site located at or near 25764 Loop 494 in Kingwood, Texas. Employees were exposed to overhead hazards when working in areas underneath other employees using nail guns on site-constructed scaffolding.

Employees were working on field constructed scaffold platforms that were not at least 18 inches in width, exposing employees to fall hazards.

On or about November 4, 2013 and times prior thereto, employees of Stanley Lantzsch Contracting performed framing and decking work at a construction site located at or near 25764 Loop 494 in Kingwood, Texas. Employees were performing decking on the second story without the use of fall protection, exposing employees to a fall distance of greater than 6 feet to a lower level.
