29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:       a.    On or about November 6, 2013 and prior thereto, at the console area of Plant 1, employees dispensing and mixing chemicals such as but not limited to methyl ethyl ketone peroxide were exposed to the hazard of splashing methyl ethyl ketone peroxide into their eyes.  Employees were wearing safety glasses with side shields that allowed chemicals to splash around the glasses.     b.    On or about November 6, 2013 and prior thereto, at the gel coat booths in Plant 1, employees spraying gel coat  containing chemicals such as but not limited to styrene were not required to wear eye protection to prevent eye contact with the chemicals.  Employees were exposed to the hazard of eye contact with styrene from overspray.     c.    On or about November 5, 2013 and prior thereto, at the Aluminum Plant, employees dispensing and mixing chemicals such as but not limited to amines,  4,4  diphenylmethane diisocyanate (MDI), and polymethylene polyphenyl isocyanate were exposed to the hazard of splashing chemicals into their eyes.  Employees were wearing safety glasses with side shields that allowed chemicals to splash around the glasses.      d.    On or about November 5, 2013 and prior thereto, at the Aluminum Plant, employees  spraying chemicals such as but not limtied to amines,  4,4  diphenylmethane diisocyanate (MDI), polymethylene polyphenyl isocyanate,  methyl ketone, methyl ethyl ketone,  1-methoxy-2-proply acetate were exposed to the hazard of eye contact with the chemicals from overspray and lines rupturing on spray equipment.    Employees were not wearing eye protection.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:          a.      On or about November 6, 2013 and prior thereto, in the storage and mixing room between the spray rooms, employees dispensing and mixing chemicals such as but not limited to amines and 4,4 diphenylmethane diioscyanate (MDI) were not provided with an eyewash station in the mixing area to flush their eyes with water in the event of  eye contact with the chemicals from a splash or spill.       b.      On or about November 6, 2013 and prior thereto, the eyewash station located on the wall opposite the spray rooms was blocked by boats located between the spray rooms and the wall.  This condition exposed employees spraying chemicals such as but not limited to amines and 4,4 diphenylmethane diisocyanate (MDI) to the hazard of not having an eyewash station available to flush their eyes with water in the event of eye contact with the chemical from  overspray.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:     On or about November 6, 2013 and prior thereto, employees at the Aluminum Plant working with chemicals such as but not limited to 4,4 Diphenyl polyphenyl isocyanate (MDI), aromatic amine, polyoxyprooplenediamine, hexamethylene di-isocyanate, methyl ethyl ketone, methyl ketone, acetone and naphtha were not provided information on the location and availability of material safety data sheets.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):     On or about August 8, 2013 and  prior thereto, at the Aluminum Plant, all employees required to wear 3M half face respirators while spraying coating containing chemicals such as but not limited to 4,4  diphenylmethane diisocyanate (MDI), and polymethylene polyphenyl isocyanate were not fit tested.
