OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards.    On or about November 6, 2013, at the work site located at Key Energy Services, Rig #66, 5750 E. I-20, Kennedale, Texas: employees assigned to work from the derrick man's work platform at approximately 25 to 30 feet were exposed to fire and explosion hazards.  The employer did not ensure the anchor stakes were flushed to the ground so that the anchor would withstand at least 3000lbs.
