29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. SW Corner of Roof for Area C Class Rooms: Employees were not protected from falls of approximately 14 feet 8 inches to the ground below as they were gathering materials and tools that were left at the unprotected edge of the roof by the operator of the rough terrain fork lift as observed on 11-5-13. b. Sky Light Area for Area C: Employees were exposed to a fall of approximately 14 feet 8 inches to the concrete floor below while accessing and climbing off the the extension ladder. The sky light tower did not have a guard rail system around all open sides as determined on 11-5-13. Abatement Certification Required for this Item

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. Sky Light Area: The employees used the blue fiberglass extension ladder to gain access to the roof deck and the side rails of the ladder only extended above the landing 16 inches in lieu of 3 feet as determined on 11-20-13. Abatement Certification is Required for this Item
