29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  The employer does not ensure that all working levels on scaffolding are fully planked or decked. This violation occurred on or about November 05, 2013, at a construction site when an employee was exposed to a fall hazard while installing electrical equipment using a scaffold without the working platform being fully planked or decked between the front uprights and the guardrail supports.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels on all scaffolds are appropriately planked or decked.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  The employer does not ensure that all supported scaffold poles, legs, posts, frames, and uprights are equipped with base plates and mud sills or other adequate firm foundation. This violation occurred on or about November 05, 2013, at a construction site where an employee was exposed to fall hazard while installing electrical equipment using a scaffold that was set up on a dirt foundation without base plates or mud sills.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds are set up on base plates and mud sills or other adequate firm foundation.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   The employer does not ensure that a guardrail system is installed on all working levels of scaffolds. This violation occurred on or about November 05, 2013, at a construction site when an employee was exposed to a fall hazard while installing electrical equipment using a scaffold without a guardrail system in place.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds used by employees have a guardrail system installed that protects employees from falling from all open sides and ends of the platform.
