29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: (a) Federal Correctional Institution Safford, 1569 West Hwy 366, Safford, AZ 85546, Salvage Yard: On or about November 1, 2013, the forklift operator was not issued nor did he wear a protective helmet to protect him from falling objects when lifting loads above his head, exposing him to hazards.

29 CFR 1910.178(m)(5)(iii): A powered industrial truck operator, dismounted and within 25 feet of the truck still in his view, did not have brakes set to prevent movement, did not put gear in neutral, and did not fully lower load engaging means, exposing employees in the vicinty to hazards: (a) Salvage Yard near Firing Range, Federal Correctional Institution Safford, AZ 85546: On or about November 1, 2013, a forklift operator dismounted from a Caterpillar RC-60 Forklift to assist another person in lifting a fallen load at a dumpster site, without setting the brakes or putting the controls into neutral, or lowering the fork tines fully to the ground, exposing employees in the proximity to hazards.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: (a) Federal Correctional Institution(FCI) Safford, AZ 85546: On or about November 1, 2013, a Caterpillar RC-60 forklift was placed into service to haul, lift and dump debris loads (plastic bales) into a dumpster located in the Salvage Yard, without any documented pre-use check conducted prior to use that day on the forklift by the operator or the supervisor, exposing these employees to hazards.
