29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about November 06, 2013, an employee was exposed to a fall hazard of more than 6 feet to the dirt ground below while performing roofing shingling activity on an 8/12 pitch roof.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:    On or about November 06, 2013, an employee was exposed to a fall hazard of more than 6 feet to the dirt ground below while using an extension ladder, which did not extend 3 feet above the landing, to access a roof.
