29 CFR 1917.45(f)(9): Live parts of electrical equipment on cranes or derricks were not located or enclosed so as to prevent accidental contact. (a) On or about November 7, 2013 the inspection revealed that employees were exposed to serious injury because live parts of electrical equipment on the American whirly crane #2 were not located or enclosed to prevent accidental contact. Crane #2 is located between the Mitsui crane and crane #3 at the north side of the Schnitzer facility adjacent to an inlet of the Willamette River at 12005 N Burgard in Portland, Oregon. The live electrical parts were located at the power distribution panel adjacent to the wire rope drum. (b) On or about November 7, 2013 the inspection revealed that employees were exposed to serious injury because live parts of electrical equipment on the American whirly crane #3 were not located or enclosed to prevent accidental contact. Crane #3 is white/grey and is located in the northeast corner of the Schnitzer facility adjacent to an inlet of the Willamette River at 12005 N Burgard in Portland, Oregon. The live electrical parts were located at the power distribution panel adjacent to the wire rope drum.

29 CFR 1917.50(c)(1): Each crane and derrick was not tested as a unit quadrennially and examined annually. (a) On or about November 7, 2013 the inspection revealed that the Mitsui/Paceco Portainer/4190 did not receive an annual inspection by an OSHA accredited inspector within the required timeframe. This crane is located at the northwest corner of the Schnitzer Facility adjacent to an inlet of the Willamette River at 12005 N Burgard in Portland, Oregon. (b) On or about November 7, 2013 the inspection revealed that the American whirly crane #2 did not receive annual and quadrennial inspections by an OSHA accredited inspector within the required timeframe. This crane is yellow and is located at the north side of the Schnitzer facility adjacent to an inlet of the Willamette River between the Mitsui/Paceco Portainer and American whirly crane #3 at 12005 N Burgard in Portland, Oregon. (c) On or about November 7, 2013 the inspection revealed that the American whirly crane #3 did not receive annual and quadrennial inspections by an OSHA accredited inspector within the required timeframe. This crane is white/grey and is located in the northeast corner of the Schnitzer facility adjacent to an inlet of the Willamette River at 12005 N Burgard in Portland, Oregon.
