B)  Center aisle of warehouse, on or about November 8, 2013, the unused sections of the iron worker blade was not guarded to prevent employees from putting their fingers or hands into the area, exposing employees to a lacerations and amputation hazards.    C) Center front section of warehouse, on or about November 8, 2013, the rear area of a shearing machine was not guarded to prevent employees from entering the area during operation.
