29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:      (a) Site wide - Employees were working with and around chemicals and a written hazard communicaiton program had not been developed or implemented.

29 CFR 1910.1200(f)(6)(ii): Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.          (a)  Maintenance shop - On or about November 17, 2013 a container of acid and a contairer of  floor cleaner was not properly labeled.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:          (a) Maintenance shop - On or about November 17, 2013 a relocatable power tap was plugged into an extension cord that was plugged into another relocatable power tap thereby potentially exposing employees to the hazard of coming in contact with an energized source.
