29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:    1.) On or about November 8, 2013, the employer failed to provide Appendix D of the respiratory protection standard to employees who wore filtering facepiece respirators (dust masks) voluntarily.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form:    1.) On or about November 8, 2013, the employer failed to conduct exposure monitoring to determine employees exposure to various metals and acids, such as, but not limited to, zinc and hydrochloric acid while working on electroplating lines.
