29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about November 7, 2013, and times prior thereto the employer did not ensure electrical equipment used at a commercial construction site was used as per the equipment's listed and labeling: 1. At the rear North East corner outside of the construction project a 120/208V 10 Gauge Romex Cord lying on the ground was utilized as an extension cord to supply electrical power to employees performing electrical installation work inside of the construction project . 2. At the rear North East corner outside of the construction project a 120/208V electrical cabinet used to supply electrical power and branch feeds electrical to employees was not provided with weather proofing or designed for outdoor use.

29 CFR 1926.403(e): All splices and joints and free ends of conductors were not covered with insulation equivalent to that of the conductors or with an insulating device designed for the purpose: On or about November 7, 2013, and times prior thereto, electrical tape was used to insulate splices on an electrical cable that provided 120 / 208 volt service to the site, exposing employees to electrical hazards.
