The wood dust accumulation was measured to be over 2 inches deep and was a fire hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: For the facility at 666 49th St., Saint Petersburg, FL 33707, as observed on or about 11/08/2013: a) A power cord for the feeder motor on the Rockford table saw had its outer insulation damaged such that strain was placed on its inner conductors. b) The power cord for the Rockford saw motor had outer insulation damage where it connected to the motor placing strain on the inner conductors.
