29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about November 5, 2013, an employee was exposed to greater than a 6 foot fall while disconnecting the clip on the lanyard from the hopper and then climbing into the man basket and also disconnecting from the man basket to climb onto the hopper to connect the lanyard. In the Alternative: 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 5, 2013, an employee was exposed to greater than a 6 foot fall while disconnecting the clip on the lanyard from the hopper and then climbing into the man basket and also disconnecting from the man basket to climb onto the hopper to connect the lanyard.
