29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) In the Machine Area; an Clausing-Colchester 17 lathe did not have work-holding device guard exposing employees to struck-by hazards. (b) In the Machine Area; an OKK VM 4II CNC vertical mill had inadequate guarding in that the door could and would be opened during operation exposing employees to struck-by hazards. (c) In the CNC Room; an OKK VP 600 CNC vertical mill had inadequate guarding in that the door could and would be opened during operation exposing employees to struck-by hazards. (d) In the CNC Room; an OKK VM7II vertical mill had inadequate guarding in that the door could and would be opened during operation exposing employees to struck-by hazards. (e) In the CNC Room; an OKK VM5 vertical mill had inadequate guarding in that the door could and would be opened during operation exposing employees to struck-by hazards.
