29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation:   On or about November 4, 2013, and at times prior thereto, in Cut Shop, Bay #2 and Assembly Shop areas horizontal band saws were used without blade guards provided to prevent contact with the operating blade.  Employees were exposed to an amputation caused by the unguarded blades.

Employees were exposed to amputation hazard.
