All hinges, handles, bolts, or other parts shall set flush with the floor or cover surface:  On or at dates prior to November 7, 2013 in the store room of Nashua General Store employees were exposed to a tripping hazard in that a floor opening cover projected more than 1 inch above floor level.  The handle was not flush with the floor.
