29 CFR 1910.23(e)(7)(iii): The construction of floor opening covers may be of any material that meets the strength requirements. Covers projecting not more than 1 inch above the floor level may be used providing all edges are chamfered to an angle with the horizontal of not over 30 degrees. All hinges, handles, bolts, or other parts shall set flush with the floor or cover surface:  On or at dates prior to November 7, 2013 in the store room of Nashua General Store employees were exposed to a tripping hazard in that a floor opening cover projected more than 1 inch above floor level.  The handle was not flush with the floor.

29 CFR 1910.303(b)(7): 29 CFR 1910.303(b)(7):Unused openings in boxes, cabinets, equipment cases, and  housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  On, or at dates prior to, November, 7, 2013, employees were exposed to a risk of fire from a Square D electrical box, located in the stairwell of the building, which had an unused opening on the right side of the cabinet.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded:  On or on dates prior to November 7, 2013 employees working at Nashua General Store were exposed to an electric shock hazard in that the lamp used in the large walk in cooler was not protected by a guard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   On, or at dates prior to, January 25, 2012, at 801 E Hwy 42, Iberia, Missouri, employees on the workroom floor were exposed to a fire hazard due to the improper use of flexible electrical cord as permanent fixed wiring of an electrical outlet.
