29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers: a. On or about November 5, 2013, at the above addressed jobsite, employees were exposed to tripping hazards due to four uncovered column holes along the walkway in the main entrance area between the buildings.
