On or about November 9, 2013, located on the roof of the building construction site, workers were exposed to the hazards of being struck by rebar in that the rebar mat was not sufficiently secured together to withstand the forces of a vertical lift being imposed on the tie wires that held it together. On or about November 9, 2013, at the site, a rebar mat was being lifted into place by a tower crane.  The tie wires broke and the rebar mat fell to the floor and broke apart striking several workers, including an employee with the tagline.  The area were the mat fell was in front of the stairway used by workers to an access the roof exposing workers to the hazards of being struck by rebar.
