29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction:  Drilling Rig:  On or about November 04, 2013, employees were working on the mud pit floor where an opening was large enough that someone could walk and was not guarded by a floor hole cover.  This condition exposed employees to a potential fall hazard.

29 CFR 1910.23(e)(6): All handrails and railings shall be provided with a clearance of not less than 3 inches between the handrail or railing and any other object.  Drilling Rig: On or about November 04, 2013 employees were working on the rig floor more than 10 feet above the ground level where they were exposed to an opening of more than 3 inches between the guardrail and the stairs.  This condition exposed employees to a potential fall hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws  At the light plant trailer: On or about November 04, 2013 employees were exposed to electrical hazards when an electrical cord did not have strain relief at the end of an opening in a junction box connecting centrifugals pumps.
