29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:    (a)   Professional Stucco & Stone, LLC at 840 Pitkin Ave, Grand Junction, CO 81501:  On and before November 8, 2013, the employer did not ensure the working platform on the fabricated frame scaffold was fully planked in that only 1or 2 planks were placed on all levels of the scaffold sections.  This condition exposed the employees to fall hazards of approximately 12-14 feet.

29 CFR 1926.451(e)(6)(i):  Prefabricated scaffold access frames shall be specifically designed and constructed for use as ladder rungs:      (a)     Professional Stucco & Stone, LLC at 840 Pitkin Ave, Grand Junction, CO 81501:  On and before November 8, 2013, the employer did not ensure that fabricated access frames were specifically designed and constructed for use as ladder rungs, in that employees climbed scaffold frames to access all levels of the scaffold used at the jobsite. The scaffold frames were not designed to be used as a scaffold ladder access. This condition exposed employees to the hazard of falling from heights of approximately 12-14 feet.

29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    (a)  Professional Stucco & Stone, LLC at 840 Pitkin Ave, Grand Junction, CO 81501:  On and before November 8, 2013, the employer did not ensure employees working on a scaffold were protected from falls via the use of guardrails along all open sides and ends of the scaffold.  This condition exposed employees to fall hazards of approximately 12-14 feet.

29 CFR 1926.451(f)(3):  Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    (a)  Professional Stucco & Stone, LLC at 840 Pitkin Ave, Grand Junction, CO 81501:  On and before November 8, 2013, the employer did not ensure the fabricated frame scaffold had been inspected for visible defects by a competent person before use.  Without the inspections employees were exposed to fall hazards of approximately 12-14 feet.    Abatement Note:  Concerning the requirements and duties of a competent person:    (1)  "Competent person" means one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.    (2)  The competent person must be able to carry out the tasks specified in the standard. In order to be able to carry out the tasks the competent person needs to have knowledge in the issues relating to the strength and structural integrity of the scaffold. A competent person must be fully knowledgeable about erecting, disassembling, moving, operating, repairing, maintaining and inspecting the scaffold.      (3)  Section 1926.454 states that a competent person will train employees involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold. To meet this requirement, a competent person must be fully knowledgeable about erecting, disassembling, moving, operating, repairing, maintaining and inspecting the scaffold.

29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration:    (a)  Professional Stucco & Stone, LLC at 840 Pitkin Ave, Grand Junction, CO 81501:  On and before November 8, 2013, the employer did not ensure a fabricated frame scaffold was dismantled with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration.  Employees assembled and dismantled a scaffold system that was up to three-levels high on all sides of the building without the supervision of a qualified competent person.  This condition exposed employees to falls of up to 12-14 feet.
