OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious phyisical harm to employees: On or about November 06, 2013, and at times prior thereto, employees were working on Rig #6 at a known hydrogen sulfide field. The employer did not ensure that the H2S monitoring system was installed and used in the work area. Employees were exposed to hazards involving hydrogen sulfide gas such as asphyxiation, explosions and/or fire.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious phyisical harm to employees: On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #6. The employer did not ensure that the anchor capacities were verified prior to attaching guywires and that the verification was less than 2 years old. One anchor was attached to a tree and others did not have verification tags. Employees were exposed to potential struck-by, crushing and structural collapse hazards.

The employees were working on the rig with railings lacking on the two ends of the platform and in the middle as well. Employees were exposed to potential fall hazards.
