The employees were working on the platforms of the rig with two railings not set up to protect against falls of 4 feet or more.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: On or about November 07, 2013, and at times prior thereto, employees were working on a rig site. The stairway used to access the platform of the rig was lacking its side rails. Employees were exposed to fall hazards of 4 feet or more.

The ladder to access the derrick had a rung that was bent extensively as to project out and cause a slip/trip hazard. Employees were exposed to falls of 4 feet or more.
