29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:    (a) On or about October 30th, 2013, and at times prior, an employee was exposed to fall hazards while installing a telecommunications device on a roof without the use of a personal fall arrest system.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about October 30, 2013, and at times prior, the employee used a ladder that did not extend 3 feet above the upper landing surface to access a roof.
