29 CFR 1926.250(b)(9): Structural steel, poles, pipe, bar stock, and other cylindrical materials, unless racked, shall be stacked and blocked so as to prevent spreading or tilting.  a. At the site located in Scio, Ohio the employer had employees that stacked steel pipes on a flatbed truck.  On or about November 1, 2013 the material was placed on the second tier of trailer and a piece of pipe was placed with a 90 degree T-fitting placed in the air which caused it to rotate and strike the foreman causing serious injury.  Employees had used a rock from the ground to block the pipes from movement.

On or about November 1, 2013 a foreman was seriously injured while this employee was signalling the movement of an approximately 4,000 pound pipe.
