29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 12, 2013, on the north side of the construction site, employees were exposed to a fall hazard of approximately 8 feet while engaged in framing activities on a commercial construction site.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about November 12, 2013, on the east side of the commercial work site, employees engaged in framing activities were not provided a ladder for access to the second level exposing employees to fall hazards of approximately 8 feet.
