29 CFR 1910.23(d)(1)(ii): Stairway(s) less than 44 inches wide having one side open, did not have at least one stair railing on the open side: (a) North side of elevator: On November 7, 2013 and at times prior, employees were exposed to slip, trip, and fall hazards while using the four riser stairs that were oriented to the west along side of the railroad tracks. The stairs were approximately 40 1/2 inches wide.

29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces: (a) East Elevator: On November 7 , 2013 and at times prior, employees were exposed to explosions, fires and/or deflagration from Class II combustible grain dust that that was approximately three to four inches deep on the overhead ledges and beams. COLUMBIA GRAIN, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.272(j)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314316456, CITATION NUMBER 01, ITEM NUMBER 004 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 18, 2011 , WITH RESPECT TO A WORKPLACE LOCATED AT #11 SOUTH MAIN STREET CHOTEAU, MT 59422.
