29 CFR 1910.1052(d)(2): The employer did not perform initial monitoring to determine each employee's exposure to methylene chloride: a) On or about November 12, 2013, and at times prior, employees were exposed to unknown concentrations of methylene chloride because the employer failed to conduct an initial exposure assessment for employees of the Ortho fuser line that used a solvent mixture, which contained 50% methylene chloride, as an adhesive for assembly of medical parts.

29 CFR 1910.305(g)(1)(iv): Flexibles cords and cables were used for purposes prohibited by subparagraphs (a) through (e) of this paragraph: a) On or about November 12, 2013, and at times prior, employees were exposed to electrical shock and fire hazards because flexible cords were attached to building surfaces and used as a substitute for fixed wiring (such as conduit) in the maintenance shop and the CVE room. Flexible cords were attached to the wall in the maintenance shop and attached to the ceiling and direct wired into standard metal outlet boxes in the CVE room.
