29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: Rig #9: On or about November 5, 2013, one employee was observed walking down the stair case located on the operator's side, the stairs were missing rails both sides. This condition exposed the employees to a trip hazard.

29 CFR 1910.157(c)(4): Portable fire extinguishers were not maintained in a fully charged and operable condition: Rig #9: On or about November 5, 2013, the employer did not maintain in operating condition the fire extinguisher located on top of the battery box behind the cab. This condition exposed the employees to fire hazards.
