29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage:  a. On June 25, 2013, employees were using an extension cord that was damaged. The cord was being used to energize a hand power saw. The employees were exposed to a contact hazard with electrical current.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:  a. On June 25, 2013, the strain relief was damaged and the wires were visible. The extension cord was being used to power an electric hand saw.

Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part:  a.  Employees working on the top plate did not demonstrate retention of requisite knowledge for fall protection. The employees were exposed to a fall hazard.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:  a.  On June 25, 2013, there were three employees climbing the framework of the house to gain access to the top plate and the trusses. The employees did not have a means off access or egress that complied with this standard, thereby exposing the employees to a fall hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities six feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a. On June 25, 2013, three employees were working on the top plate of a two story, single family home without fall protection. The employees were exposed to a fall hazard.  Buckeye Framing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 836523, citation number 1, item number 3 and was affirmed as a final order on 02 April, 2013, with respect to a workplace located at 6852 Marigny Avenue building 24, New Albany, OH 43054.
