At this job site, a large, multi-family residential project, employees worked from a personnel platform which was elevated by a forklift. The personnel platform was not a manufactured platform available for purchase or rental; the platform was fabricated for employee use from angle iron. There were no markings on the platform to indicate the platform had been designed by a qualified person.

1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.   At this multi-family residential project under construction, a personnel platform was not securely attached to the forklift. A rope had been used to loosely attach the platform to the forklift.
