29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Global Precision Parts, Inc., located at 13060 State Route 287, East Liberty, OH 43319: On or about November 12, 2013, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of the A-26 Acme-Gridley screw machine (Model 3 1/2 RB-6, Serial # 94672). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, setting-up the machine and changing tooling. Employees were exposed to amputation hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were exposed to injuries from unexpected start-up of the machine and energy release. A) Global Precision Parts, Inc., located at 13060 State Route 287, East Liberty, OH 43319: On or about November 12, 2013, the employer did not ensure that load engaging means of the Clark forklift (Model CGC25, Serial # C365L00499545FB) were fully lowered when the forklift was parked in the production area of the facility while employees were using it to load 12 feet long steel bars into screw machines. Employees were exposed to lacerations, fractures, and broken bones hazards if an employee would walk into the forks of the powered industrial vehicle, as well as to crushing and struck-by hazards if the load engaging means of the forklift would fail while employees are working in the immediate proximity. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
