29 CFR 1910.132(a): Protective equipment was not used when necessary and whenever hazards capable of causing injury and impairment were encountered: a) On or about November 7, 2013, the employer, Rocky Mountain Food Company, LLC, did not ensure employees, who were required to cook food using steam-heated vats supplied by pressurized steam pipes, were supplied with personal protective equipment so as to prevent physical contact with, and subsequent burns from steam pipes.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: a) On or about November 7, 2013, the employer, Rocky Mountain Food Company, LLC, did not ensure employees, who were required to operate and clean the rotating auger machine supplied by 240 volts, were protected from electrical shock in that fittings covering electrical conductors which entered a cutout box were not closed and secured.
