On or about November 13, 2013, two employees using Hitachi NR83A2 pneumatic air nail guns were not wearing safety glasses, exposing themselves to being hit by nails or flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about November 13, 2013, on a 6:12 pitch roof, three employees performing roofing operations were not protected from a 10 feet 4-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a. On or about Novemeber 13, 2013, employees used a Werner extension ladder to access and egress a roof, that did not extend 3 feet over the eave, exposing employees to a 10-foot fall hazard.
