29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section. On or about November 05, 2013, and prior thereto, employees used the front and rear flight of stairs having more than four risers to access the workover rig platform. The stairs were not equipped with standard handrails, exposing employees to a fall hazard.
