Job site located at 15202 Wycliffe Dr, Omaha, NE 68154-Employees re-roofing an apartment building were exposed to a fall hazard of over 25 feet without wearing fall protection. Most of the employees did have a full body harness on, but were not tied off properly with a rope and rope grab system.
