29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   (a) The two wrappers and the oven are hard-wired pieces of equipment that are maintained and repaired by employees. There is no energy control procedure, training, periodic inspections, lockout tagout hardware, or equipment specific procedures for locking out and tagging out equipment where the unexpected startup or release of stored energy could occur and cause injury.   Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  (a) Near the outfeed side of the oven, adjacent to the main aisle, the end door panel of the oven had been removed, exposing employees to three rotating projecting shafts.    Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Downstream of the grinder, adjacent to where the masa comes out by conveyor, a metal junction box with prepunched knockouts intact, was being used as a pendent fourplex receptacle. Employees were exposed to electrical hazards.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) The facility uses hazardous chemicals such as Zep Amine A, a disinfectant/sanitizer, without having developed, implemented or maintaining a written hazard communication program at the workplace. Zep Amine A can cause central nervous system depresion, burns to the eyes and skin, respiratory tract irritation, and chronic exposure can dammage the middle ear.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
