Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) On or about November 13, 2013, at a jobsite located at 3412 S. 106th Street, Chicago, IL. - employees performing roofing activities on a roof with a 4:12 pitch were not protected from falling by the use of personal fall arrest systems, thereby exposing the employees to fall hazards of approximately twelve (12) feet eight (8) inches.
